Not all problems can be solved with medicine. Our Social Prescriber is here to support patients with non-medical needs that affect their health and wellbeing.
We can help if you’re:
- Living with a long-term condition
- Struggling with low mood, stress, or mild mental health concerns
- Feeling lonely, isolated, or disconnected
- Dealing with housing, money, or family-related issues
Our social prescriber will work with you 1-to-1 to help you get the support you need. That might include connecting you with:
- Local social groups or activities
- Support for managing your health
- Help with housing, employment, or finances
- Specialist services and community organisations
You’ll usually work with a social prescriber over several appointments across a few months. It’s completely free, and tailored to you.
